Your answers and the screening result are processed only in the open page. They are not submitted to HawkNest, included in contact emails, or saved by the check after you close or reload the page.

Requesting contact

When the callback form is active, it collects the contact details you provide, your preferred contact method, optional location and referral source, and permission to respond. It tells Adam that you completed the check and requested contact; it does not include your answers or result. Share your own details, not another person’s medical information.

Contact requests are sent through FormSubmit and delivered by email to adam@hawknestfallsafety.com. FormSubmit may use cookies and security checks. See FormSubmit’s privacy terms. Submitting a request does not sign you up for marketing messages.

Other services

Booking opens Google Calendar. The Facebook link opens Facebook. Those services handle information according to their own policies. This site does not add advertising pixels or send screening answers to advertising services. The hosting provider may process technical connection data needed to deliver the site.
